On September 2, 2025, ISBE issued amended and one new Special Education Required Notice and Consent Forms (Form 34-57A-P). ISBE further provided a chart explaining the changes and updates to the notice and consent forms. (link to explanation chart).
There are multiple changes.
Parent/Guardian Notification of Decision Regarding a Request for an Evaluation Form 34-57A
The amended Form now requires documentation of a district’s rationale for granting or denying requests for initial evaluations and reevaluations.
Parent/Guardian Identification of Needed Assessment Forms 34-57B-C
The amended Form requires documentation of parental consent to waive draft materials when IEP meetings are scheduled within less than three days of the meeting (Parent/Guardian Consent for an Initial Evaluation Form 34-57B and C). Parent/Guardian Consent for Reevaluation Form 34-57C is modified to align domains with the language of ED Regulations. ISBE also restructured the domain categories for identifying needed assessments to align with ED regulations.
Parent/Guardian Notification of Conference Form 34-57D
The Form is amended to document the date by which materials will be delivered or made available to parents ahead of IEP meetings. The Form also removes the prior language regarding convening an “assessment planning meeting.”
Parent/Guardian Consent for Initial Provision of Special Education and Related Services Form 34-57F
ISBE removed the requirement for drafting IEPs and convening IEP meetings ahead of informed parental consent for initial IEP evaluations, making it consistent with ED regulations.
Parent/Guardian and Student Notification of Transfer of Rights Due to Age of Majority Form 34-57I
ISBE moved up notification of the majority for students who turn seventeen during summer vacation to the last school session (Form 34-57I).
Parent/Guardian Notification of Conference – Identification of Needed Assessments Form 34-57P
ISBE created a new mandate for Notification of Conference Identifying Needed Assessments, implementing ISBE’s position regarding the necessity of a meeting to develop the “domains” for an evaluation. Scheduling conferences to identify student assessment needs ahead of revaluation and initial IEP meetings.
